BOARDING

Please fill out the Boarding Policies Document Attached.

You only need to fill out one document per stay, unless you have different instructions for multiple pets.

Day Admission / Drop-off

Please fill out the Day admission / Drop-off form for each pet, each visit.

You will need your account number to complete the form.  You can find it on the top of your last invoice, or we can fill that in at the Hospital.  If you are going to drop-off a pet for any type of illness or skin problem, please be as detailed as possible in describing the problems.  This will help the Doctor(s) in their diagnostic process.
